Community Based Palliative Care Conference

 

Institute on Aging invites you to join us for this full day conference to cover many aspects of Palliative Care in the Community. 

The program will address a variety of emotional, spiritual and practical issues that families and older adults face at the end-of-life.  The conference will cover topics including: Advance Care Planning, Client-Directed Care Considerations at the End of Life, and Zen Hospice’s Dying Well. Covering many aspects of end of life services in the San Francisco Bay Area, this conference strives to go in depth on the topic of Palliative Care, and its role in the community, specifically outside of the hospital setting.

 

Course Objectives:

  1. Implement advance care planning during all phases of patient care
  2. Describe the uses and practical implications of the terms comfort care and palliative care
  3. Define and describe how to implement person-centered care (including physical, emotional, cultural and spiritual considerations) at end of life.
  4. Facilitate conversations with clients and family members to clarify goals of care in light of the individual client’s specific preferences.
  5. Apply the principles and practices of cultural humility and self-care as health professionals working with diverse clients at the end of life.
